Neuromyelitis optica (NMO) is an autoimmune relapsing inflammatory disorder of the central nervous
system. The detection of AQP4 antibody in NMO helped the recognition of various atypical clinical
presentations including hyperCKemia. We report an anti-AQP4 antibody positive NMOSD patient
presented with hyperCKemia and dematomyositis-like clinical features.
