Paroxysmal focal dystonia is an uncommon clinical manifestation of multiple sclerosis (MS). We present the case of 37-year-old woman with paroxysmal focal hand dystonia as the only initial symptom of MS. Brain MRI showed a focal enhancing lesion in the internal capsule and multiple demyelinating lesions. Findings of cerebrospinal fluid analysis and follow-up MRI were compatible with prototype MS. When paroxysmal focal hand dystonia is the only symptom of disease, MS should be included in the differential diagnosis. Journal of Multiple Sclerosis 1(2):64-66, 2010
