Longitudinal Extensive Transverse Myelitis Caused by Tuberculosis Infection

Longitudinal extensive transverse myelitis (LETM) due to tuberculosis is uncommon. A 65-year-old
man presented with clinical and radiologic features of LETM. Cerebrospinal fluid (CSF) analysis revealed
prominent pleocytosis, increased protein, decreased glucose and increased adenosine
deaminase. The patient was diagnosed with tuberculosis LETM and treated with steroid and anti-tuberculosis
medications. His symptoms and neurologic deficits disappeared and follow-up
whole-spine magnetic resonance imaging was normal. Tuberculosis meningitis should be considered
in cases of LETM when the CSF profile of a patient with LETM is abnormal.
